Key Responsibilities of the Travel Home Health Physical Therapist role: Perform comprehensive assessments including patient history, musculoskeletal and neurological exams, functional mobility, balance, strength, range of motion. Identify barriers to safe function and home environment needs. Create measurable, patient-specific therapeutic goals. Utilize therapeutic exercises, manual therapies, gait/training, balance/vestibular retraining, ADL/environmental adaptations, modalities as appropriate. Provide fall prevention strategies and home safety education. Reassess patient progress regularly; adjust goals and interventions. Collaborate with home health nurses, occupational therapists, speech therapists, physicians. Prepare documentation for regulatory compliance, reimbursement, and discharge procedures. Train patients/caregivers in HEP (home exercise program), functional transfers, proper use of assistive devices, environment safety. Maintain a full caseload, traveling to homes within Lebanon and surrounding townships. Manage schedule, documentation, and planning efficiently.
