Makes daily rounds on all residents. Ensures that immediate
action is taken on clinical, environmental and safety issues.
Follows up with the charge nurses, physicians, and other
departments as necessary in resolving issues pertaining to the
residents and/or the staff.Reviews the 24-hour report daily.
Directs and ensures action is taken on identified problems from the
previous shifts.Oversees the admissions process, including a
nursing inquiry for planned admissions. Ensures that all admission
paperwork, including the residents' personal inventories, is
completed by nursing. Assists in formulating an interim care plan
as needed. Collaborates with social work in facilitating the
resident's adjustment to new surroundings.Maintains and monitors
clinical record documentation according to current clinical
practice guidelines. This includes, but is not limited to:Daily
review and follow-up on laboratory/radiology reports.Review of
consultant's reports.Review of physician orders/new
orders/end-of-month orders and carries out those orders, or
delegates this to the charge nurse.Oversee nursing documentation
and educates the charge nurses on appropriate follow-up of clinical
conditions, as well as documentation of the care plan goals for
each resident.Monitor physician visits according to Collington
guidelines.Communicate advance directive information to the charge
nurses.Perform chart audits on new admissions as indicated.Monitors
narcotics books and emergency equipment for signatures.Update
resident's care plans accordingly with any clinical changes and
attend the interdisciplinary meetings. Confers with the Director of
Nursing and MDS Coordinator daily concerning the on-going clinical
and administrative management of the nursing unit. May confer with
the Assisted Living Manager concerning the on-going clinical and
administrative management of the assisted living units
also.Participates in weekly wound rounds, oversees the charge
nurses' documentation of a weekly skin assessment on all residents.
Provides education as required to the staff in skin integrity
maintenance.Coordinates the discharge planning process by
interaction with the residents, families, and social workers, as
required by the care plan and/or the residents' clinical condition.
Advises the receiving facilities, either verbally or in writing, of
the residents' care needs.Acts as a clinical resource and role
model to the nursing staff under his/her direct supervision. Always
maintains a professional demeanor and ensures accountability for
professional practice.Assumes other related duties as assigned by
the Director of Nursing.
